The Initial Preventive Physical Examination (IPPE) — also known as the “Welcome to Medicare Preventive Visit” — is a preventive visit offered once within 12 months of first Part B enrollment. Despite its name, the IPPE does not include an extensive physical examination, but rather focuses on review of your medical and social health history and preventive services education.

If you've had Medicare Part B (outpatient coverage) for more than 12 months, you can schedule an Annual "Wellness" visit every 12 months to develop and subsequently update a personalized prevention plan with your provider. The visit is intended to help you maintain good health by catching signs of disease early on and taking a preventive approach to healthcare.

If you’re transitioning from an acute care setting or emergency room visit, you’ll receive hospital follow-up with your TRI-MED primary care provider. These visits allow our providers to review medication changes and test results following hospital and other acute care encounters, as well as provide a chance to evaluate new treatment options. The goal is to prevent repeat events and keep you healthy and out of the hospital.

TRI-MED offers student and adult physical examinations required for participation in sports programs: $35 for youth and $85 for adults. We also offer physicals required for employer and nursing physicals.

TRI-MED provides annual well-woman preventive care for adult women (age and developmentally appropriate), including cervical cancer screening, preconception care, and many services necessary for prenatal care. Other prenatal care and screening services necessary for optimal women’s health are referred to specialty providers.

As a Georgia Family Planning System (GFPS) family planning service site, TRI-MED provides access to high-quality pregnancy planning and prevention services integrated with other preventive health care for women, men, and adolescents.

Tri-Med Family Care is committed to practicing integrated health care and recognizes that both medical and behavioral health factors are important parts of a person’s overall health. Medical and behavioral health clinicians work together as a team to address a patient’s concerns. It is a core function of the “advanced patient-centered medical home.”

TRI-MED is credentialed as a Community Provider in the VA Community Care Network (CCN), the VA’s direct link with community providers to ensure Veterans receive timely, high-quality care. In most cases, you must receive approval from the VA before receiving care from a community provider to avoid being billed for the care. VA staff members generally make all eligibility determinations for community care.

Finding retinal disorders as early as possible is critical to potentially preventing serious disease progression and even vision loss. Retinal eye exams help providers assess the health of your retina and detect eye and health conditions such as diabetes, glaucoma, and macular degeneration.

There is no need to visit another facility for diagnostic laboratory testing. Quest staff work on-site to draw blood and collect other samples that are delivered to Quest laboratories for testing or tested at TRI-MED during your visit. Once lab results are returned to us, your healthcare provider will review these results with you over the phone or during a follow-up visit.

We offer chronic care management services to our Medicare patients who live with two or more serious chronic conditions such as arthritis, asthma, diabetes, hypertension, heart disease, and osteoporosis. Our clinicians will prepare a comprehensive care plan that lists your health challenges and goals, other providers you see, your medications, community services you have and need, and other information about your health. It also explains the care you need and how your care will be coordinated. You will also receive support when you go from one health care setting to another, a review of your medications and how you take them, and help with other chronic care needs.

TRI-MED administers vaccines to protect you from the following dangerous or deadly diseases: influenza (regular and high dose vaccines), tetanus, diphtheria, and pertussis (Tdap vaccine), hepatitis B, pneumonia, and shingles.

Our on-site pharmacy participates in the 340B Drug Discount Program, a federal government program requiring drug manufacturers to provide outpatient drugs at significantly reduced prices. These prescription drug price savings are directly passed onto you as a Tri-Med patient, regardless of insurance status.

We are proud to offer the assistance of a Federally Certified Marketplace Navigator. Our on-site navigator helps you enroll in coverage through the Health Insurance Exchange and assists with Medicaid enrollment. If you are currently uninsured, we can help you find the right health coverage. If you are insured, but need help making a change, we can assist you during open and special enrollment periods.

As a Federally Qualified Health Center (FQHC), we are here to serve patients regardless of income or insurance status.
